India
Public route: Government hospitals � AIIMS, NIMHANS, district hospitals � provide developmental assessments at minimal or no cost. The quality of assessment varies significantly by institution and by the specific team available. In major cities, government specialist units are often excellent. In smaller cities and rural areas, the infrastructure is thinner.
The DEIC (District Early Intervention Centre) system under RBSK provides free developmental screening and referral for children through government schools and Anganwadis. This is worth exploring if you are in a smaller city.
Private route: Developmental paediatricians in private practice in major cities can see children within a few weeks. A full assessment including ADOS-2, developmental history, and report typically costs ?8,000�?15,000 in metro cities. Paediatrician fees may be separate.
Can I use a private diagnosis for the RPWD disability certificate?
The RPWD Act disability certificate must be signed by a government hospital physician � typically from the Disability Assessment Board at a district hospital or above. However, the diagnosis can be made by a private specialist whose full assessment report is submitted to the government hospital. In practice, many families get a thorough private assessment first and then use that report at a government hospital for the certificate. The government hospital may do a brief review rather than a full reassessment.
Insurance: Very few Indian health insurance policies currently cover developmental assessments or autism therapy. This is improving slowly � some group health plans now include limited mental health coverage. Worth checking your specific policy.
United Kingdom
NHS route: Completely free, but waiting times are the most significant barrier in the UK. Average waiting times from GP referral to completed assessment are currently 18 months to 3 years in most areas, though this varies significantly by clinical commissioning group. Some areas have specialist CAMHS teams with shorter waits for young children.

Waiting while waiting: Apply for an EHCP while waiting for the assessment. You do not need a formal diagnosis to request one, and the process takes 20 weeks once started. Starting it now means it is in progress when the diagnosis comes.
United States
Early Intervention (under 3): Completely free. Contact your state's Early Intervention programme � you can self-refer, no GP needed. The evaluation must be completed within 45 days. This is one of the most accessible public early childhood services globally.
School-based evaluation (3+): Free through the public school district under IDEA. Make the request in writing. The school must complete the evaluation within 60 days (or the state's timeline).

What to Do While You Wait
Waiting for an assessment is frustrating, but the time is not wasted if you use it well.
- Document everything now. Start a detailed log of what you observe � behaviours, developmental milestones reached and missed, video clips. This information is exactly what the assessment will ask for, and having it organised saves time and improves the quality of the assessment.
- Start home-based support activities. You do not need a diagnosis to begin supporting your child's communication and development at home. Naturalistic play-based activities, visual schedules, and joint attention games are beneficial regardless of outcome.
- Contact your child's school or nursery. Teachers can often start implementing supportive strategies before a diagnosis. In most countries, you do not need a diagnosis to request accommodations.
- Connect with a parent support network. Other families who have been through the process in your specific area often have the most practical local knowledge.
